Email Statement Disclosure and Electronic Consent Agreement
This agreement made between you and Park Bank is an addendum to the Park Bank Online Banking User Terms and Conditions disclosure and agreement previously agreed upon. By completing this consent form and agreement you authorize Park Bank to electronically deliver your monthly Statement of Account.
By enrolling in E-Statements, you have requested and Park Bank has agreed to notify you via the e-mail address you provided to the Bank, notice that all of your bank account statements and other information are electronically available with the Bank’s online banking program. Park Bank will send, electronically to the same e-mail address, a message containing a notice of how to view available information by accessing Park Bank’s online banking or a message containing a link to www.ParkBankOnline.com where the information is located. There is no fee to receive your statements electronically.
If your Park Bank account is owned jointly with another person(s), any one of you may consent to receive electronic disclosures and E-Statements and that person’s election to access E-Statements shall apply to all of you and you agree to receive regulatory and disclosure notices through the Bank’s website.
By receiving your statement electronically, you acknowledge that you will not receive a hard copy of your bank account statement or any images of cancelled checks. Your electronic bank account statement will provide the number of each check that has cleared, the amount of each check, and the date of payment posting. If you request a hard copy of either your bank statement or a check referenced in that statement, normal research fees may apply. If you request a copy of a check from the Bank, the Bank will provide a legible copy of the check within a reasonable period of time.
Access to your E-Statements
An email notification will be sent to you at the address provided by you to let you know when your E-Statement is available for viewing. Your E-Statement for your current statement cycle will be available no later than 3 business days after your cycle date. Your normal statement cut-off date will not change when you elect to receive E-Statements. E-Statements will be available for viewing by you for 18 months after the date it is posted on the on-line banking website as long as your account and online banking remain active during that time. You may find it advisable to print or download E-Statements for permanent retention.
Technical Requirements
To access and retain your E-Statements you need the following: 1) Internet access and an Internet e-mail account and address; 2) Online Banking through Park Bank; 3) Internet Explorer 6.0 or higher; 4) Adobe Acrobat Reader 8.0 or higher; 5) Access to a printer or the ability to download information in order to keep copies of your E-Statements for your records.
How to Discontinue Electronic Delivery of E-Statements
At any time you may revoke your consent to electronic delivery and instead receive paper statements. There is no fee if you elect to receive paper statements. You may also contact us at the address list below. (See “How to Contact Park Bank” below)
Password Security
Your user ID and password that are used to access on-line banking services should be kept confidential. Do not disclose or share your password with any third party. You are responsible for keeping your password, account numbers, and other account data confidential.
Your Responsibility to Notify Us Regarding Errors, Irregularities, or Unauthorized Access

We must hear from you no later than 60 days after we sent the first statement on which the problem or error appeared.
- Tell us your name and account number (if any).
- Describe the error or the transfer you are unsure about, and explain as clearly as you can why you believe it is an error or why you need more information.
- Tell us the dollar amount of the suspected error.
If you tell us orally, we may require that you send us your complaint or question in writing within 10 business days.
We will tell you the results of our investigation within 10 days after we hear from you and will correct any error promptly.
If we need more time, however, we may take up to 45 days to investigate your complaint or question. If we decide to do this, we will recredit your account within 10 business days for the amount you think is in error, so that you will have the use of the money during the time it takes us to complete our investigation. If we ask you to put your complaint or question in writing and we do not receive it within 10 business days, we may not recredit your account.
If the error involves an electronic transfer from your account to buy goods or services, or a transfer initiated outside of the United States, the time period for the actions stated will be 10 business days and 90 days in place of 45 days.
If we decide that there was no error, we will send you a written explanation within 3 business days after we finish our investigation. You may ask for copies of the documents that we used in our investigation.
Your Responsibility to Update Your Electronic Contact Information
We will send all statements, notices, attachments, etc. via e-mail to the last known address provided by you. You agree to notify the Bank if there is a change in your e-mail address or any other information needed to contact you electronically. (See “How to Contact Park Bank” below) You may contact the Bank as shown below or through your Internet banking site, see “Customer Service” in the Bank’s on-line banking site.
If a disclosure or notice provided by e-mail is returned to the Bank, Park Bank will make every effort to re-deliver the notice to you based on information we have about you.
Fees for E-Statements
Park Bank does not currently charge fees for E-Statement delivery. We do reserve the right to change or impose fees for electronic delivery and charge your deposit account for these fees at any time after providing notice to you as required by law.
Amendment and Termination of the E-Statement Agreement by Park Bank
We may amend or change the E-Statement Agreement at any time and we will provide to you any notice or request for authorization required by law or regulation. If no notice or request for authorization is required by law or regulation, your continued acceptance of E-Statements after the effective date of such change will constitute your acceptance of and agreement with such amendment(s).
We may terminate this Agreement and immediately refuse to provide, or revoke access to E-Statements at any time with or without cause or prior notice. If we terminate your access to our on-line banking service you will no longer receive E-Statements.
